Women’s Care 

The Community Health Representative’s (CHR) provide education in pre-natal, post-natal, and infant care to citizens and spouses of the Peoria Tribe. Included in this education is the growth progress-through weight and length measurements.

CHR’s will connect tribal citizens with the Inter-tribal WIC program. The Inter-tribal WIC program can assist women, infants and children. Services they provide are food assistance, nutrition education and health care referrals.

CHR’s also provide education to our female tribal population on techniques for breast self-exams. This provides early detection for signs of cancer.

Health Education

Our Community Health Representative’s provide in-office and/or at-home health education for tribal citizens. This includes:

  • Well-being Assessments (social, physical, and mental health)
  • Healthy Lifestyle Management Education
  • Companionship; provide in-home visits and medication assistance.

General Health Services

The Community Health Representative bridges the gap between resources and health needs. CHR’s provide resources for transportation to and from appointments, and medication pick-up and drop-off.

Nutritional Services

Community Health Representatives will provide nutrition education and outreach services to Tribal citizens. Nutrition education is a vital part of a comprehensive health education program and empowers Tribal citizens with the knowledge and skills to make healthy food and beverage choices. Outreach activities will include nutritional classes with a focus on healthy meal alternatives for families.

Substance Abuse/Behavioral Services/Peer Support

Peer Recovery Support Specialists can effectively extend the reach of treatment beyond the clinical setting into the everyday environment of those seeking a successful, sustained recovery journey.

Counseling and referral services for all ages are made available through the Substance Abuse Prevention Program.
